🔴 PRIORITY 1: ESSENTIAL (Fill out right away)

  • date_YYYYMMDD: Today's date (20250823)
  • time_HHMM: When it happened (1415 = 2:15 PM)
  • volunteer_name: Your full name
  • animal_name: The animal's name
  • location: Where you were (Kennel 5, Play Yard, etc.)
  • what_happened_notes: Simple description - just write what you saw!

🟡 PRIORITY 2: IMPORTANT (Fill same day when possible)

  • activity_context: What were you doing? (walking, feeding, playing)
  • trigger_what_started_it: What happened right before?
  • behavior_observed: More specific - barking, jumping, hiding, etc.
  • intensity: Just write: low, med, or high
  • duration_estimate: About how long? (30 seconds, 2 minutes, etc.)
  • actions_taken: What did you do to help?

⚪ PRIORITY 3: DETAILED (Complete later or with staff)

  • volunteer_initials: Your initials (JS, MK, etc.)
  • animal_id: Official ID number if known
  • distance_to_trigger_ft: How far from trigger? (estimate in feet)
  • recovery_time_seconds: How long to calm down?
  • enrichment_tried: Toys, treats, activities used
  • staff_notified: Which staff member did you tell?
  • follow_up_needed_YN: Does this need follow-up? (Y/N)
  • follow_up_details: What kind of follow-up?
  • photo_video_refs: Names of any photos/videos taken
