🔴 PRIORITY 1: ESSENTIAL (Fill out right away)
- date_YYYYMMDD: Today's date (20250823)
- time_HHMM: When it happened (1415 = 2:15 PM)
- volunteer_name: Your full name
- animal_name: The animal's name
- location: Where you were (Kennel 5, Play Yard, etc.)
- what_happened_notes: Simple description - just write what you saw!
🟡 PRIORITY 2: IMPORTANT (Fill same day when possible)
- activity_context: What were you doing? (walking, feeding, playing)
- trigger_what_started_it: What happened right before?
- behavior_observed: More specific - barking, jumping, hiding, etc.
- intensity: Just write: low, med, or high
- duration_estimate: About how long? (30 seconds, 2 minutes, etc.)
- actions_taken: What did you do to help?
⚪ PRIORITY 3: DETAILED (Complete later or with staff)
- volunteer_initials: Your initials (JS, MK, etc.)
- animal_id: Official ID number if known
- distance_to_trigger_ft: How far from trigger? (estimate in feet)
- recovery_time_seconds: How long to calm down?
- enrichment_tried: Toys, treats, activities used
- staff_notified: Which staff member did you tell?
- follow_up_needed_YN: Does this need follow-up? (Y/N)
- follow_up_details: What kind of follow-up?
- photo_video_refs: Names of any photos/videos taken
